The National Transportation Safety Board (NTSB) has found that Atlas Air flight 3591 crashed outside Houston in 2019 after the first officer, due partly to sensory illusion, improperly reacted to inadvertent activation of the aircraft’s go-around mode. Investigators also concluded the captain’s failure to command a positive transfer of control of the airplane as soon as he attempted to intervene on the controls enabled the first officer to continue to force the airplane into a steepening dive.”On top of that, investigators found that the first officer had performance deficiencies which he took deliberate steps to conceal, and might have led Atlas Air to take appropriate action had they known of them in the first place.“The first officer in this accident deliberately concealed his history of performance deficiencies, which limited Atlas Air’s ability to fully evaluate his aptitude and competency as a pilot,” said NTSB As a result of the investigation, the NTSB has issued six new safety recommendations to the FAA, addressing “flight crew performance, industry pilot hiring process deficiencies, and adaptations of automatic ground collision avoidance system technology.”Gabriel Leigh grew up on long-haul flights and has been fascinated by airplanes since he can remember. The report shows from ADS-B data that the plane was following a normal descent and arrival procedure into the airport moments before the accident took place.The data shows the plane descending through 13,800 feet at which point the controller in charge of the aircraft advised the crew of light to heavy precipitation front that was moving along their initially planned flight route and so they should expect to be given vectors around the weather for their arrival.The pilots were then asked if they would like to be vectored to the north or the west of the weather for their arrival into the airport on runway 26L.The data shortly after this new approach was agreed shows the plane continuing to descend through 12,000 feet with a ground speed of 290 knots, which the report states are consistent with the arrival procedure of the airport.A few minutes later the pilots were instructed by ATC to turn to a heading of 270 degrees, the ADS-B data shows that this command was followed out by the crew and at the point of taking the heading the plane was still in a decent passing through 8,500 feet.The ADS-B data goes on to show that the plane and pilots were complying with all ATC instructions. This incident resulted in three fatalities. However, the NTSB also points to the captain’s failure to properly monitor the situation, as well as failings in industry standards for performance assessment that failed to catch “aptitude related deficiencies and maladaptive stress response” on the part of the first officer. Monday, August 10, 2020 Investigators from the FAA, Federal Bureau of Investigation (FBI), and National Transportation Safety Board (NTSB) were dispatched to the accident site with the NTSB leading the accident investigation.
